Quality of Life Grant Application

(All applicants must review MDRT Foundation grant application guidelines and eligibility requirements before downloading an application.)

The Quality of Life Grant Program recognizes outstanding volunteerism by MDRT members on behalf of a charitable organization. Their efforts are commended and the volunteers selected receive a Quality of Life Award in honor of their service to improve the quality of life for those less fortunate. Quality of Life grants are tiered and start at $2,500 (up to 20 awards), $5,000 (3 awards), $10,000 (2 awards) and $25,000 (one award).

Applicant (MDRT Member) Eligibility

  • Applicant must be an MDRT member in the year that the grant is awarded.
  • Only one Quality of Life grant (other than the top six) may be awarded to any individual in a three-year period.
  • Upon receiving one of the top six Quality of Life awards, an applicant may no longer apply under the Quality of Life program. (Applicant may apply under a different MDRT Foundation program, subject to eligibility.)
  • Applicant may only apply for, or endorse, one application per year for all grant programs, subject to eligibility.
  • Applicant must be a contributor to the MDRT Foundation.
