a new life insurance policy
To
enroll in the Legacy of Life insurance program with a new life insurance policy, please submit the following
to the MDRT Foundation:
- Life insurance
policy application
- Ledger outlining
premium schedule with a vanishing point no later than after 10 years
- Check made out
to the Million Dollar Round Table Foundation (who will in turn issue
a check to the insurance company)
Mail all three to:
